Background:

Benign lobular granulomatous mastitis (LGM) presents a diagnostic challenge, often clinically resembling breast carcinoma.
Understanding LGM's variable presentation and treatment is crucial for accurate diagnosis and patient management.

Observation:

Two cases of LGM are presented, both initially suspected to be malignant based on clinical examination.
Histological features were consistent with previously reported cases of LGM.

Findings:

Immunological assessment revealed a predominance of stromal T lymphocytes and intra-epithelial T lymphocytes within ducts.
One patient achieved remission through local excision of the affected mass.
The second patient responded successfully to treatment with oral prednisolone.

Implications:

These cases underscore the importance of considering LGM in the differential diagnosis of breast lesions.
The successful outcomes with distinct treatment modalities (surgical vs. medical) demonstrate the variable therapeutic strategies for LGM.
Further research into the immunological underpinnings of LGM may refine diagnostic and therapeutic approaches.
