Summary

A rare case of lung cancer metastasis to the kidney shows adenocarcinoma cells within glomeruli, mimicking membranoproliferative glomerulonephritis. This suggests a novel pathway for tumor cell spread and kidney damage.

Background:

  • Lung cancer can present with paraneoplastic syndromes affecting the kidneys.
  • Nephrotic syndrome and renal dysfunction are known complications of various cancers.

Observation:

  • An 88-year-old man with lung cancer, nephrotic syndrome, and renal dysfunction experienced sudden cardiac death.
  • Autopsy revealed adenocarcinoma cells within glomerular capillaries (neoplastic cell interposition) and membranoproliferative glomerulonephritis-like changes in the kidneys.

Findings:

  • Intraglomerular tumor cell metastasis was observed, characterized by neoplastic cell interposition.
  • Glomeruli exhibited lobular transformation, GBM doubling, and electron-dense deposits, resembling membranoproliferative glomerulonephritis.
