Summary

Interscalene brachial plexus block (IBPB) may cause temporary hearing loss in the same ear. This auditory dysfunction, observed in some patients, might be linked to sympathetic nerve effects.

Background:

  • Interscalene brachial plexus block (IBPB) is a common regional anesthesia technique for shoulder surgery.
  • Potential side effects of IBPB, particularly auditory changes, require further investigation.

Purpose of the Study:

  • To investigate the incidence and characteristics of auditory dysfunction following interscalene brachial plexus block.

Main Methods:

  • Audiometric threshold measurements (0.25-18 kHz) were conducted before IBPB, 2-6 hours post-surgery, and on the first postoperative day in 20 patients.
  • Patients received bupivacaine 0.75% with adrenaline followed by a 24-hour continuous infusion of 0.25% bupivacaine.

Main Results:

  • Four patients exhibited hearing impairment on the side of the block post-operation.

  • Impairment was noted on the day of surgery in three patients and the following day in one.
  • Maximum threshold change was 35 dB at 6 kHz; one patient also showed contralateral hearing changes.
  • Conclusions:

    • Interscalene brachial plexus block may induce transient auditory dysfunction in the ipsilateral ear.
    • The mechanism may involve an effect on sympathetic innervation, warranting further research into anesthetic-related ototoxicity.
