Background:

Thyroglossal duct cysts and ectopic thyroid glands can present as midline neck masses.
Misidentification of ectopic thyroid tissue as a thyroglossal duct cyst can lead to its inadvertent surgical removal.
This complication can result in iatrogenic hypothyroidism.

Observation:

A case report of a 5-year-old girl with a midline neck mass initially diagnosed as a thyroglossal duct cyst.
The mass was confirmed to be an ectopic thyroid gland.

Findings:

Ectopic thyroid glands require careful differentiation from thyroglossal duct cysts.
Embryologic review highlights the developmental pathway of ectopic thyroid tissue.
