Cutaneous vasculitis can develop in patients with plaque psoriasis, a rare complication not previously documented. In three observed cases, vasculitis resolved spontaneously, often without systemic effects or need for immunosuppression.

Area of Science:

  • Dermatology
  • Immunology

Background:

  • Cutaneous vasculitis is a known complication of pustular psoriasis.
  • Its occurrence in plaque-type psoriasis has not been previously reported.

Observation:

  • Three patients with plaque psoriasis developed cutaneous vasculitis over a 5-year period.
  • Two patients had psoriatic arthropathy and were on etretinate; vasculitis did not worsen on this medication.
  • The third patient was not on retinoids.

Findings:

  • No specific cause for vasculitis was identified in these patients.
  • Skin lesions resolved spontaneously in most cases.
  • One patient required surgical amputation due to vasculitis complications.
  • No cases required immunosuppressive therapy.

Implications:

  • This study highlights cutaneous vasculitis as a potential, albeit rare, complication of plaque psoriasis.
  • The exact trigger for vasculitis in these cases remains unknown but is suspected to be immune-mediated.
  • Further research is needed to elucidate the pathogenesis and optimal management strategies.
