Summary

Hyperthyroidism can cause bile acid malabsorption, leading to diarrhea. Treating the thyroid condition improved both symptoms and bile acid absorption, suggesting a direct link.

Background:

  • Hyperthyroidism, particularly Graves' disease, can present with gastrointestinal symptoms.
  • Diarrhea is a common but often multifactorial symptom in thyrotoxicosis.

Observation:

  • A 40-year-old male with Graves' disease experienced dyspepsia, watery diarrhea, and weight loss.
  • Diagnostic tests confirmed hyperthyroidism and revealed significant bile acid malabsorption, indicated by low SeHCAT retention.

Findings:

  • Antithyroid treatment with carbimazole and propranolol normalized thyroid hormone levels.
  • Concurrently, the patient's diarrhea resolved, and bile acid absorption improved significantly (SeHCAT retention increased to 14.7%).

Implications:

  • This case strongly suggests that bile acid malabsorption is associated with hyperthyroidism.
