    Summary

    Technetium-99m-sestamibi (Tc-99m-sestamibi) scanning aids in locating abnormal parathyroid glands, especially in reoperative cases. Increasing the Tc-99m-sestamibi dose to 15 mCi significantly improves scan sensitivity for hyperparathyroidism.

    Background:

    • Preoperative localization of abnormal parathyroid glands remains a challenge.
    • Technetium-99m-sestamibi (Tc-99m-sestamibi), initially used for cardiac imaging, is now applied to parathyroid scintigraphy.

    Purpose of the Study:

    • To evaluate the efficacy of Tc-99m-sestamibi scanning for preoperative localization of abnormal parathyroid glands in patients with hyperparathyroidism.

    Main Methods:

    • A retrospective analysis of 45 Tc-99m-sestamibi scans performed on 44 patients with hyperparathyroidism between August 1991 and September 1993.
    • Patient cohort included diverse cases: persistent, recurrent, and primary hyperparathyroidism, as well as multiglandular disease and parathyroid carcinoma.

    • The overall sensitivity of Tc-99m-sestamibi scans was 62% for all abnormal glands, increasing to 79% when at least one abnormal gland was detected.
    • Increasing the injected dose to 15 mCi (in the last 20 patients) significantly improved sensitivity to 80% (all glands) and 90% (at least one gland) (p = 0.03).
    • Scans were more effective in identifying mediastinal (75%) versus cervical (58%) abnormal glands and in single-gland disease.

    Conclusions:

    • Tc-99m-sestamibi scanning is a valuable adjunct for parathyroid localization, particularly in reoperative settings.
    • Higher injected doses of Tc-99m-sestamibi enhance diagnostic sensitivity.
    • The technique shows promise for identifying abnormal parathyroid tissue in complex cases of hyperparathyroidism.
