Summary

A rare case of blue-grey macular pigmentation appeared in a young boy, presenting with clinical and histological features resembling idiopathic macular pigmentation. Further ultramicroscopic analysis may link it to lichen planus.

Area of Science:

  • Dermatology
  • Pediatric Dermatology
  • Histopathology

Background:

  • Hyperpigmentation presents diverse etiologies, including idiopathic forms.
  • Understanding the spectrum of pediatric hyperpigmentation is crucial for accurate diagnosis.

Observation:

  • A 4-year-old boy developed blue-grey macular lesions (2-3 cm diameter) on the trunk, neck, and proximal legs over 5 months.
  • Lesions were asymptomatic, without pruritus or urtication upon rubbing.
  • Histopathology revealed epidermal acanthosis, hyperkeratosis, and dermal melanophage infiltration.

Findings:

  • The clinical presentation and 6-month stable course mimic idiopathic macular pigmentation typically seen in adolescents.
  • Histological findings of melanophage infiltration are characteristic of certain pigmentary disorders.
  • Ultramicroscopic examination is suggested for definitive classification, potentially linking to lichen planus.

Implications:

  • This case highlights a rare presentation of macular pigmentation in a pre-adolescent child.
  • The findings underscore the importance of integrating clinical, histological, and potentially ultrastructural data for diagnosing pigmentary disorders.
  • Further research into the pathogenesis and classification of such idiopathic macular pigmentations is warranted.
