Background:

  • Scleritis is a destructive ocular disease often associated with autoimmune disorders.
  • The role of primary vasculitis in scleritis pathogenesis is suspected, but cellular mechanisms remain unclear.
  • Understanding the inflammatory cellular infiltrate is crucial for elucidating scleritis pathogenesis.

Observation:

  • Episcleral biopsies from nodular scleritis patients and enucleated eyes from necrotizing and posterior scleritis cases were analyzed.
  • Morphological and immunohistochemical methods characterized the inflammatory cells.
  • The study examined cellular infiltrates in various forms of scleritis, including anterior necrotizing and posterior scleritis.

Findings:

  • The primary inflammatory cells in scleritis were T lymphocytes and macrophages, with a CD4+ T cell predominance.
  • Few lymphocytes expressed the IL-2 receptor, indicating limited activation.
  • Necrotizing scleritis showed granulomatous features with activated macrophages (epithelioid and giant cells).
  • While T cells dominated the infiltrate, B cells were found in perivascular areas, and neutrophils, macrophages, and plasma cells were present focally.
  • Major histocompatibility class II (MHC II) molecules were expressed on lymphocytes and some macrophages.
  • No definitive signs of primary vasculitis were observed in the studied specimens.

Implications:

  • The cellular infiltrate in scleritis exhibits characteristics consistent with a T cell-mediated autoimmune disorder.
  • These findings suggest potential autoimmune mechanisms driving scleritis pathogenesis.
  • Understanding these cellular dynamics may lead to significant therapeutic advancements for scleritis patients.
