Background:

Clear cell acanthoma (CCA) is a benign epithelial skin tumor.
A rare pigmented variant of CCA has been described.

Observation:

Five cases of macroscopically pigmented clear cell acanthomas were analyzed.
Masson-Fontana silver stain identified numerous dendritic melanocytes with melanin granules within tumoral keratinocytes.
These cells expressed S-100 protein.

Findings:

Electromicroscopy revealed abundant melanosomes in melanocyte dendrites.
A decreased number of melanosomes in adjacent keratinocytes suggests impaired melanin transfer.
This variant is proposed to be named 'pigmented clear cell acanthoma'.

Implications:

This study characterizes a rare pigmented variant of clear cell acanthoma.
Understanding melanin transfer disturbances in this condition is crucial for diagnosis.
The findings contribute to the classification of skin tumors.
