Tacalcitol in psoriasis: a video-microscopy study

Summary

Video-microscopy allows in vivo visualization of skin capillaries. This study shows tacalcitol treatment improves psoriasis by reducing capillary tortuosity and dilation.

Background:

Video-microscopy enables direct in vivo visualization of skin surface and capillaries.
This technique is valuable for studying normal skin microcirculation and detecting capillary changes in skin diseases like psoriasis.

Purpose of the Study:

To evaluate the in vivo morphology of capillaries in psoriatic plaques.
To assess the effects of topical tacalcitol, a vitamin D3 analogue, on these capillaries.

Main Methods:

Utilized video-microscopy to observe skin capillary morphology.
Patients with psoriasis were treated with topical tacalcitol for 6 weeks.
Clinical evaluations and capillary morphology assessments were performed at 3 and 6 weeks.

Main Results:

After 3 weeks of tacalcitol therapy, a reduction in erythema and scaling was observed, with less tortuous capillaries becoming apparent.
By 6 weeks, significant improvements were noted: capillaries in the psoriatic plaques were less dilated and tortuous.
The characteristic large and tortuous appearance of active psoriasis capillaries diminished after treatment.

Conclusions:

Topical tacalcitol effectively improves the microcirculation in psoriatic plaques.
The study demonstrates tacalcitol's ability to normalize abnormal capillary morphology associated with psoriasis.
Video-microscopy is a useful tool for monitoring treatment response in psoriasis.
