Summary

A patient with a history of melanoma experienced vision loss due to pigmented melanoma cells dispersed throughout the anterior segment of his eye. Pathologic examination confirmed melanoma cells without a discrete intraocular tumor.

Background:

  • Cutaneous melanoma is a significant risk factor for ocular metastasis.
  • Anterior segment involvement by melanoma can present with varied clinical signs.

Observation:

  • A 63-year-old male with a history of melanoma presented with progressive vision loss and significant anterior segment pigmentation.
  • Clinical and echographic evaluations did not reveal an intraocular tumor.
  • Pigment dispersion was noted in the aqueous humor and anterior vitreous.

Findings:

  • Pathologic examination identified numerous heavily pigmented melanoma cells diffusely distributed in the anterior segment.
  • Melanoma cells were primarily concentrated in the anterior vitreous near the ciliary body.
  • No discrete intraocular tumor mass was identified on globe sectioning.
