Summary

Prophylactic amnioinfusion in term patients with oligohydramnios did not improve perinatal outcomes compared to indicated use. Amnioinfusion is best reserved for laboring patients with variable fetal heart rate decelerations.

Area of Science:

  • Obstetrics
  • Maternal-Fetal Medicine
  • Perinatal Outcomes

Background:

  • Oligohydramnios, a condition of low amniotic fluid, can complicate term pregnancies.
  • The role of amnioinfusion in managing oligohydramnios during labor is debated.
  • Standard care involves therapeutic amnioinfusion only when variable fetal heart rate decelerations occur.

Purpose of the Study:

  • To compare the efficacy of prophylactic amnioinfusion versus standard care with indicated therapeutic amnioinfusion in term patients with oligohydramnios.
  • To assess the impact on cesarean delivery rates, fetal distress, and intrapartum complications.

Main Methods:

  • A randomized controlled trial involving 116 term gestations with oligohydramnios (amniotic fluid index < 5.0 cm).
  • Patients were assigned to either prophylactic amnioinfusion (600-mL bolus + 3 mL/minute) or standard care (control).

Related Experiment Videos

  • Control patients received therapeutic amnioinfusion for moderate/severe variable decelerations.
  • Main Results:

    • No significant differences were observed in overall cesarean delivery rates (21% vs. 17%) or cesarean delivery for fetal distress (7% vs. 10%) between groups.
    • The prophylactic amnioinfusion group showed a significantly higher rate of intrapartum fever (23% vs. 7%).
    • Only 22% of control patients developed variable decelerations requiring therapeutic amnioinfusion.

    Conclusions:

    • Prophylactic amnioinfusion does not offer benefits over indicated amnioinfusion for perinatal outcomes in term oligohydramnios.
    • Amnioinfusion should be reserved for term laboring patients experiencing variable fetal heart rate decelerations, not all cases of oligohydramnios.
