Summary

Pyriform sinus fistulas can cause recurrent cervical abscesses and acute suppurative thyroiditis in children and young adults. Complete surgical removal of the fistula is the recommended treatment.

Background:

  • Pyriform sinus fistulas serve as a route for infection into the perithyroidal space and thyroid gland.
  • Direct fistula communication with the thyroid gland can result in primary acute suppurative thyroiditis.
  • Chronic cervical draining sinus with a history of repeated drainage procedures is a key diagnostic indicator.

Implications:

  • Early diagnosis and complete surgical extirpation of the fistula are crucial to prevent recurrent infections.
  • Barium paste swallow studies are recommended for diagnosis.
  • Understanding this pathway is vital for managing pediatric neck infections and thyroiditis.
