Summary

Simplified hip arthroscopy without traction is effective for diagnosis and treatment in most cases. However, the conventional technique with traction is still needed for specific joint surface and acetabular fossa issues.

Context:

Hip arthroscopy is a minimally invasive procedure for diagnosing and treating hip joint pathologies.
Traditional hip arthroscopy often utilizes traction to visualize joint surfaces, which can be cumbersome.

Purpose:

To evaluate the diagnostic accuracy and therapeutic efficacy of a simplified hip arthroscopy technique performed without traction.
To compare the outcomes of the simplified method with the conventional traction-assisted approach.

Summary:

A retrospective review of 225 hip arthroscopies (228 simplified, 15 traction) was conducted.
Patient questionnaires assessed diagnostic and therapeutic outcomes, with follow-up averaging 3 years.
The simplified method showed low error rates (5% normal, 22% indeterminate) and satisfactory therapeutic results in 40% of chondromatosis cases, though repeat procedures were sometimes needed.

Impact:

The simplified hip arthroscopy technique is proposed as a routine procedure for hip disorders.
Conventional traction arthroscopy remains essential for visualizing congruent cartilage surfaces and acetabular fossa lesions identified by imaging like CT scans.
