Summary

Pronounced bronchial hyper-responsiveness in asthma patients does not correlate with disease severity. A simplified bronchial hyper-responsiveness test is not useful for identifying severe asthma cases.

Background:

  • Asthma severity assessment is crucial for patient management.
  • Bronchial hyper-responsiveness (BHR) is a hallmark of asthma.
  • The clinical significance of BHR in relation to asthma severity requires further investigation.

Purpose of the Study:

  • To evaluate the association between pronounced bronchial hyper-responsiveness and clinical asthma severity.
  • To determine if a simplified BHR test can identify patients with severe asthma.

Main Methods:

  • Fifty-six asthma patients underwent bronchial provocation testing with methacholine chloride at low concentrations.
  • Asthma severity was retrospectively and prospectively assessed using treatment intensity, hospitalization, intensive care unit (ICU) admissions, emergency visits, and sick leave days.

    • Pronounced bronchial hyper-responsiveness was observed in 28 out of 56 patients.

    • Pronounced bronchial hyper-responsiveness is not a reliable indicator of asthma severity in a clinical setting.
    • A single, simplified bronchial hyper-responsiveness test, without considering disease status or medication, is insufficient for identifying patients with severe asthma or predicting a poor prognosis.
