White matter lesions (leuko-araiosis) in the frontal lobe are more common in older adults and linked to brain atrophy and reduced blood flow, potentially predicting cognitive decline.

Conclusions:
- Cortical atrophy and hypoperfusion/ischemia in frontal white matter contribute to frontal leuko-araiosis in normal aging.
- Frontal leuko-araiosis may serve as an early indicator of future cognitive decline.
