Area of Science:

  • Otolaryngology
  • Infectious Diseases
  • Diabetology

Background:

  • Malignant external otitis (MEO) is traditionally associated with diabetes mellitus.
  • The role of diabetes in the pathogenesis of MEO requires further investigation.

Purpose of the Study:

  • To identify and characterize a cohort of nondiabetic patients diagnosed with malignant external otitis.
  • To challenge the traditional belief linking MEO exclusively to diabetes.

Main Methods:

  • Retrospective analysis of 30 patients diagnosed with MEO between 1987 and 1991.
  • Clinical and laboratory data review, focusing on diabetic status and Pseudomonas aeruginosa infection.

Main Results:

  • Nine out of 30 MEO patients (30%) showed no evidence of diabetes.

Related Experiment Videos

  • Nondiabetic MEO patients presented with severe ear canal pain, closure, granulation tissue, and resistance to treatment.
  • Pseudomonas aeruginosa was identified as a common pathogen.
  • Conclusions:

    • Malignant external otitis should be considered in nondiabetic patients with Pseudomonas aeruginosa ear infections.
    • Specific clinical signs like severe pain, canal obstruction, and granulation tissue are indicative in nondiabetic MEO cases.
