Summary

Electroconvulsive therapy (ECT) combined with theophylline may increase seizure duration in some patients. However, ECT can be safely administered to most severely depressed patients with appropriate precautions.

Background:

  • Electroconvulsive therapy (ECT) is a critical treatment for severe depression.
  • Theophylline, used for pulmonary conditions, poses a risk of prolonged seizures during ECT initiation.
  • Balancing the need for ECT with theophylline use presents a clinical challenge.

Observation:

  • A review of nine ECT courses in seven patients taking theophylline was conducted.
  • Seizure durations were monitored using electroencephalography (EEG).

Findings:

  • Out of 77 seizures, one lasted 190 seconds; all others were under 100 seconds.
  • Mean motor and EEG seizure lengths were comparable to existing literature.
  • Theophylline coadministration is identified as a risk factor for prolonged seizures during ECT.
