Background:

  • Diffuse Lewy body disease (DLBD) is a neurodegenerative disorder.
  • Cases have been primarily reported by Japanese authors, with limited description in France.

Observation:

  • A 56-year-old man presented with apathy and memory disturbances.
  • Symptoms progressed to aphasia, stereotypies, and parkinsonian rigidity.
  • The patient died three years after disease onset.

Findings:

  • Post-mortem examination revealed diffuse senile plaques and tangles in the cortex.
  • Strikingly widened Lewy bodies were observed in the pons and cerebral cortex (frontal, temporal, cingular).
  • These Lewy bodies were located in the 5th cortical layer and exhibited immature, irregular shapes.

Implications:

  • This case contributes to the understanding of DLBD presentation outside of Japan.
  • The findings highlight the neuropathological features of DLBD, including cortical Lewy bodies.
  • Further research is needed to elucidate the role of these lesions in clinical manifestations.
