Systemic lupus erythematosus with multiple myeloma

H Pehamberger, E Diem, K Konrad

    Acta Dermato-Venereologica

    Summary

This case report details a rare instance of co-occurring systemic lupus erythematosus and IgG multiple myeloma in a 44-year-old man. The simultaneous diagnosis of these two distinct autoimmune and plasma cell disorders is unprecedented in medical literature.

    Area of Science:

    • Immunodermatology
    • Hematology
    • Rheumatology

    Background:

    • Systemic lupus erythematosus (SLE) is a chronic autoimmune disease affecting multiple organ systems.
    • Multiple myeloma is a cancer of plasma cells, typically affecting the bone marrow.
    • Cutaneous manifestations are common in SLE but rare in multiple myeloma.

    Observation:

    • A 44-year-old man presented with an erythemato-squamous facial rash.
    • Diagnostic workup confirmed systemic lupus erythematosus via histopathology, immunofluorescence, and immunoelectron microscopy.
    • Subsequent investigations revealed an IgG type multiple myeloma.

    Findings:

    • The patient was diagnosed with both systemic lupus erythematosus and IgG multiple myeloma.
    • Histopathology, immunofluorescence (including antinuclear antibodies and lupus band test), and immunoelectron microscopy confirmed SLE.
    • The multiple myeloma showed no cutaneous involvement.

    Implications:

    • This case represents the first reported instance of the simultaneous occurrence of SLE and multiple myeloma.
    • The findings may prompt further investigation into potential shared etiological factors or immune dysregulation pathways.
    • Understanding the interplay between these conditions could inform diagnostic and therapeutic strategies for patients with overlapping autoimmune and plasma cell disorders.
