Ovarian extraembryonal teratoma. I. Endodermal sinus tumor

G K Jimerson, J D Woodruff

American Journal of Obstetrics and Gynecology
    |January 1, 1977

    Summary

Pure endodermal sinus ovarian tumors are aggressive malignancies in young women, often fatal despite early detection. Combination chemotherapy shows promise for improving outcomes in these rare ovarian cancers.

    Area of Science:

    • Gynecologic Oncology
    • Reproductive Endocrinology
    • Developmental Biology

    Background:

    • Endodermal sinus tumors (ESTs) are rare germ cell tumors of the ovary.
    • These tumors exhibit extraembryonal differentiation, posing diagnostic and therapeutic challenges.

    Purpose of the Study:

    • To report on 41 cases of pure endodermal sinus ovarian tumors.
    • To support the extraembryonal nature of ESTs through comparative studies.
    • To review clinical presentation, outcomes, and therapeutic options.

    Main Methods:

    • Retrospective case series analysis of 41 patients with pure ESTs.
    • Comparative studies using early human implantations to investigate tumor origin.
    • Review of clinical data, treatment modalities, and patient outcomes.

    Main Results:

    • Pure ESTs present as virulent malignancies predominantly in young women.
    • Tumors are often unilateral and encapsulated at diagnosis but have a nearly universally fatal outcome.
    • Limited therapeutic experience exists, but combination chemotherapy shows potential.

    Conclusions:

    • Endodermal sinus tumors are aggressive ovarian malignancies with a poor prognosis.
    • Vigorous combination chemotherapy is suggested as a promising therapeutic strategy.
    • Further research into optimal treatment protocols for ovarian ESTs is warranted.
