Background:

  • Undifferentiated carcinomas (UCs) present diagnostic challenges.
  • Immunohistochemistry aids in characterizing tumor cell differentiation.

Purpose of the Study:

  • To investigate the expression profile of cytokeratins (CKs), vimentin, and p53 protein in undifferentiated carcinomas (UCs).
  • To evaluate the utility of these markers in the diagnosis and differentiation of UCs.

Main Methods:

  • Immunohistochemical analysis of 28 UCs using antibodies against CKs (CK8, 18, 19, 5/6, 20, AE1/AE3), vimentin, p53 protein, c-erbB-2 protein, and CEA.
  • Conventional histopathology and electron microscopy were used for initial diagnoses.

  • High frequency of CK8 (61%), CK18 (61%), and CK19 (82%) expression in UCs.
  • CK5/6 was positive in 32% and CK20 in 11% of cases.

  • Vimentin and CK18 were more frequently expressed in UCs compared to squamous cell carcinomas.
  • P53 protein overexpression showed similar rates in UCs and squamous cell carcinomas, suggesting it's not a differentiation marker.

    • A combination of cytokeratin antibodies (CK cocktail) is recommended for diagnosing UCs.
    • Vimentin and specific CK profiles can aid in distinguishing UCs.
    • P53 protein overexpression is not a reliable marker for differentiation in these tumor types.
