Background:

  • Pulmonary embolism (PE) is infrequently diagnosed in pediatric populations, suggesting potential underestimation of its true incidence.
  • While etiological factors mirror those in adults, recent emphasis has been placed on the contributions of clotting factor deficiencies and oral contraceptive use.

Observation:

  • Case 1 involved a 15-year-old immobilized for an ankle strain who developed deep vein thrombophlebitis and inferior vena cava thrombus, complicated by fatal pulmonary embolism despite anticoagulation; she had antithrombin III deficiency and used oral contraceptives.
  • Case 2 presented a 15-year-old with thoracic pain and pulmonary embolism, diagnosed with hypogastric vein thrombosis; she was pregnant and managed with anticoagulation and inferior vena cava filter placement after therapeutic pregnancy termination.

Findings:

  • Adolescents experiencing pulmonary embolism may have underlying congenital clotting factor deficiencies.
  • Oral contraceptive use and pregnancy are significant risk factors for venous thromboembolism in adolescents.
  • Pulmonary embolism is a serious complication arising from femoral and pelvic vein thrombi in adolescents.

Implications:

  • Increased vigilance for pulmonary embolism is warranted in adolescents presenting with risk factors such as clotting disorders, oral contraceptive use, or pregnancy.
  • Early diagnosis and management of venous thromboembolism are crucial in pediatric patients to prevent severe outcomes.
  • Further research into the specific risk profiles and optimal management strategies for pediatric pulmonary embolism is needed.
