    Bilateral intrathoracic meningoceles, rare spinal fluid sacs in the chest, can be mistaken for other masses. Myelography is crucial for accurate diagnosis in patients with a neurofibromatosis family history.

    • Myelography is identified as the definitive diagnostic imaging modality.
    • This highlights the limitations of standard imaging techniques.

    Implications:

    • Accurate differentiation of intrathoracic meningocele from other masses is critical for appropriate management.
    • Myelography should be considered in cases with suspected intrathoracic meningocele, especially with relevant family history.
    • Improved diagnostic strategies are needed for rare mediastinal pathologies.
