Neuropeptide Y (NPY) levels decrease with standing in healthy individuals and heart failure patients, while catecholamines increase. Elevated NPY and catecholamines are found in heart failure, but do not correlate.

Background:

  • Neuropeptide Y (NPY) is typically co-released with catecholamines during major cardiovascular stress.
  • The relationship between NPY and catecholamine release during minor stresses is less understood.

Purpose of the Study:

  • To investigate plasma NPY-like immunoreactivity (NPY-Li) and catecholamine responses to orthostatic stress (standing) in normal subjects and congestive heart failure (CHF) patients.
  • To examine the effects of short-term vasodilator therapy on NPY-Li and catecholamines in CHF patients.

Main Methods:

  • Measured plasma NPY-Li and catecholamine (norepinephrine, epinephrine) levels before and after 10 minutes of standing in 8 normal subjects and 11 CHF patients.
  • Administered 6-hour vasodilator therapy (nitroglycerin) to 13 CHF patients and monitored neurohumoral levels and mean arterial pressure.

Main Results:

  • Standing decreased NPY-Li in both normal subjects and CHF patients (P < 0.01).
  • Standing increased catecholamines (norepinephrine and epinephrine) in both groups (P < 0.001).
  • Basal NPY-Li and catecholamine levels were significantly elevated in CHF patients, but did not correlate with each other. Vasodilator therapy reduced NPY-Li (P < 0.01) without altering other neurohumoral levels.

Conclusions:

  • Basal circulating NPY-Li and catecholamine levels are elevated in congestive heart failure.
  • NPY may play a role in the increased peripheral resistance observed in CHF patients.
