Area of Science:

  • Dermatology
  • Pathology

Background:

  • Epidermolytic hyperkeratosis (EH) is a genodermatosis characterized by a specific histologic finding.
  • While often associated with ichthyosis, EH can manifest in other clinical contexts.

Observation:

  • A 57-year-old man presented with hundreds of light brown, slightly atrophic papules on his back.
  • Clinically, the lesions mimicked seborrheic keratoses.
  • Histologic examination revealed epidermolytic hyperkeratosis.

Findings:

  • The patient had no personal or family history of ichthyosiform disorders.
  • Diagnosis of disseminated epidermolytic acanthoma was established based on clinical, histologic, and historical data.
  • This rare condition is identifiable through clinicopathologic correlation.

Implications:

  • Disseminated epidermolytic acanthoma is a distinct entity that requires differentiation from other conditions featuring epidermolytic hyperkeratosis.
  • Accurate diagnosis is crucial to avoid misclassification and ensure appropriate patient management.
