Summary

Pregnancy can trigger acute attacks of synovitis, a painful joint inflammation, in women with chondrocalcinosis. This condition, also known as pseudogout, may be linked to underlying metabolic or genetic factors.

Background:

  • Chondrocalcinosis is a condition characterized by the deposition of calcium pyrophosphate dihydrate (CPPD) crystals in cartilage.
  • CPPD crystal deposition disease, or pseudogout, can manifest as acute inflammatory arthritis.
  • Predisposing factors for CPPD deposition include metabolic disorders, genetic predisposition, and joint trauma.

Observation:

  • This report describes four women experiencing acute synovitis during pregnancy.
  • Two cases were confirmed as pseudogout, and two were presumptive.
  • For three patients, these were their first episodes of synovitis.

Findings:

  • Pregnancy appears to be a potential trigger for acute pseudogout attacks in susceptible individuals.
  • All patients had identified risk factors for premature CPPD deposition.
  • This is the first documented report linking pregnancy to the provocation of pseudogout.

Implications:

  • The findings suggest a novel mechanism for crystal shedding during pregnancy.
  • Understanding this association may aid in the management of crystal arthropathies in pregnant patients.
  • Further research is warranted to elucidate the precise mechanisms involved in pregnancy-induced pseudogout.
