Background:

Cesarean section rates are rising globally.
Understanding the role of primary elective cesarean sections (PECS) is crucial for modern obstetric practice, particularly in developing countries like Nigeria.
Limited data exists on the safety and efficacy of PECS in the Nigerian context.

Purpose of the Study:

To analyze the outcomes of primary elective cesarean sections (PECS) at University College Hospital, Ibadan.
To determine the place of PECS in modern obstetric practice in Nigeria.
To assess maternal and perinatal morbidity and mortality associated with PECS.

Main Methods:

Retrospective analysis of case records of 44 patients who underwent primary elective cesarean section.
Data collection included maternal and perinatal outcomes, and complications.
Study conducted at University College Hospital, Ibadan, Nigeria.

Main Results:

A total of 13,692 deliveries were reviewed, with 2,764 (22%) cesarean sections, including 63 (2.3%) primary elective sections.
Analysis of 44 PECS cases revealed no maternal deaths and low, non-serious maternal morbidity.
One perinatal death occurred, with low and less serious perinatal morbidity compared to maternal morbidity. No uterine scar ruptures were recorded in subsequent pregnancies.

Conclusions:

Primary elective cesarean section, when used judiciously, can be a safe procedure with low maternal and perinatal risks in Nigeria.
PECS can potentially reduce difficult labor and associated mortality and morbidity in Nigeria and other developing countries.
Improved socioeconomic conditions and health education play a significant role in obstetric outcomes and the appropriate utilization of procedures like PECS.
