Piggyback lenses in keratoconus

Summary

This study shows that a new aspheric piggyback lens system effectively manages rigid contact lens intolerance in keratoconus patients. Most patients experienced improved comfort and vision, with some even showing corneal healing.

Background:

  • Keratoconus often leads to intolerance of rigid contact lenses due to corneal changes or poor fit.
  • Soft bandage lenses can improve tolerance for rigid lenses worn over them.

Purpose of the Study:

  • To evaluate the efficacy of a novel aspheric piggyback lens system for keratoconus patients intolerant to rigid lenses.

Main Methods:

  • A consecutive series of 26 patients (40 eyes) with keratoconus and rigid lens intolerance were fitted with the new aspheric combination lens.
  • Patient outcomes were assessed based on discomfort, wearing time, visual acuity, and corneal condition.

Main Results:

  • 87.5% of eyes (35/40) achieved satisfactory comfort, wearing time, and visual acuity.
  • Four eyes showed corneal improvement, negating the need for continued piggyback lens wear.
  • 12.5% of eyes (5/40) experienced failures, including giant papillary conjunctivitis, severe keratoconus, and handling issues.

Conclusions:

  • The aspheric piggyback combination lens offers a successful alternative for rigid lens-intolerant keratoconus patients.
  • This approach may help delay or avoid surgical interventions in select cases.
