Summary

Transient ischemic attacks (TIAs) are diverse. A specific "capsular warning syndrome" involves recurrent internal capsule ischemia, often leading to stroke, and resists common therapies.

Area of Science:

  • Neurology
  • Vascular Neurology

Background:

  • Transient ischemic attacks (TIAs) represent a heterogeneous group of cerebrovascular events.
  • Understanding TIA subtypes is crucial for accurate diagnosis and targeted treatment.

Observation:

  • A distinct TIA subset, termed "capsular warning syndrome," was identified in 50 patients over 15 years.
  • These TIAs exclusively affected the internal capsule region, presenting with crescendo ischemic episodes.
  • This syndrome accounted for 4.5% of all TIAs and 33% of subcortical TIAs.

Findings:

  • The primary mechanism is suspected to be hemodynamic phenomena affecting small penetrating vessels.
  • Associated infarcts were typically lacunar, involving single penetrating vessels.
  • No evidence of artery-to-artery or heart-to-artery embolism was found.

  • Implications:

    • The
    • capsular warning syndrome
    • has a characteristic clinical presentation and pathophysiological basis.
    • Early recognition is vital due to a high rate (42%) of subsequent capsular stroke.
    • This syndrome highlights the need for specialized diagnostic and therapeutic approaches in cerebrovascular disease.
