Background:

  • High myopia presents significant challenges for refractive correction.
  • Traditional photorefractive keratectomy (PRK) may have limitations in treating extreme refractive errors.

Purpose of the Study:

  • To evaluate the safety and efficacy of a multi-step excimer laser PRK procedure for correcting high myopia.
  • To assess refractive outcomes and corneal changes following multi-step PRK.

Main Methods:

  • Nineteen human eyes with high myopia (>-8.00 D) underwent simultaneous two- or three-step PRK.
  • Preoperative and 6-month postoperative refractions were recorded.
  • Keratometry and pachometry were used to assess corneal changes.

Main Results:

  • Preoperative refraction averaged -12.28 D, with attempted correction of 7.83 D.
  • Six months post-surgery, mean refraction improved to -2.23 D.
  • Corneal flattening and thinning were observed; 17 eyes showed clear or trace corneal haze.

Conclusions:

  • Multi-step PRK appears to be a safe and relatively efficient option for high myopia correction.
  • Long-term follow-up is essential to determine the stability of refractive outcomes.
