Background:

  • Pancreatic ascites is a rare complication of chronic pancreatitis, characterized by high-protein, amylase-rich fluid accumulation in the peritoneum.
  • It often results from pseudocyst rupture or pancreatic duct disruption, with fewer than 250 reported cases.

Observation:

  • A study treated six patients with pancreatic ascites, five of whom were males with a history of chronic alcoholism and no prior pancreatic disease.
  • Diagnosis involved ascitic fluid analysis and endoscopic retrograde cholangiopancreatography (ERCP) to identify the leakage site.

Findings:

  • Initial treatment with total parenteral nutrition (TPN) was administered to all patients.
  • Medical therapy failed in five patients, necessitating surgical intervention in three cases.

Implications:

  • Pancreatic ascites should be considered in patients with refractory ascites.
  • A combined strategy of medical management and surgical intervention can lead to improved outcomes for pancreatic ascites.
