Summary

Background:

Oxytocics are commonly used in obstetric care to manage labor.
Assessing the safety and efficacy of oxytocic regimens in diverse healthcare settings is crucial.
Developing countries often face unique challenges in obstetric practice and resource availability.

Purpose of the Study:

To analyze the use of oxytocics during labor in three developing countries.
To evaluate potential negative effects of oxytocic treatment regimens.
To investigate the association between oxytocic use and adverse neonatal and fetal outcomes.

Main Methods:

A prospective multicenter study was conducted in Benin, Congo, and Senegal.
Data were collected from 457 to 1048 deliveries per district over 3-6 months.

Partograms monitored labor progress; data included risk factors, stillbirths, and neonatal resuscitation.

Main Results:

Oxytocics were used in 4.4-21.5% of normal labors.
Neonatal resuscitation incidence was significantly higher with oxytocic use in normal labor (RR 1.9-5.6).
Stillbirth rates were higher, though not always significantly, with oxytocic use in normal labor (pooled RR 1.9).

Conclusions:

Oxytocic use in normal labor is associated with increased risks of adverse neonatal and fetal outcomes.
The safety of obstetric treatments like oxytocics is dependent on the quality of care provided.
Careful consideration of indications and healthcare setting is vital for safe oxytocic administration.
