Background:

  • Anterior dislocation of the radial head is a known injury.
  • Posterior interosseous nerve palsy can occur acutely after elbow trauma.
  • Delayed onset of nerve palsy is uncommon.

Observation:

  • A patient presented with delayed posterior interosseous nerve palsy 39 years post-injury.
  • The radial head remained unreduced from the initial anterior dislocation.
  • Nerve compression was identified at the arcade of Frohse.

Findings:

  • Surgical intervention included radial head resection and release of the arcade of Frohse.
  • The patient showed progressive recovery of nerve function starting 6 weeks post-operation.
  • The posterior interosseous nerve was compressed and narrowed at the arcade of Frohse due to the chronic radial head dislocation.
