Client assessment tool: a means to enhance nurse-client-physician collaboration
Abstract:
The fast pace of a headache clinic prompted the authors to develop chart forms which covered all aspects of history and treatment specific to headache clients. The effort proved to be beneficial in promoting better communication between caregivers and clients due to ease and thoroughness of documentation. Use of such a tool may promote cost effectiveness, better nurse-client-physician relationships and ultimately improved client outcomes.
