Severe hypersensitivity reactions can occur with 5-hydroxytryptamine(3) (5-HT(3)) receptor antagonists, even when switching between drugs. This suggests a potential drug class effect, cautioning against switching anti-emetic medications after any prior reaction.

Background:

  • 5-hydroxytryptamine(3) (5-HT(3)) receptor antagonists (granisetron, ondansetron, tropisetron) are effective anti-emetics for chemotherapy.
  • Common side effects include headache and constipation; severe hypersensitivity reactions are rare.

Observation:

  • Two patients experienced severe hypersensitivity after switching 5-HT(3) receptor antagonists.
  • These reactions followed prior mild to moderate hypersensitivity symptoms to a different 5-HT(3) antagonist.

Findings:

  • Literature search indicates these are the first reported cases of cross-reactive hypersensitivity with 5-HT(3) antagonists.
  • This suggests a potential drug class effect among these anti-emetic medications.
