Summary

Effort capacity in mitral stenosis (MS) patients does not correlate with disease severity determined by Doppler echocardiography. This study found no significant link between exercise performance and mitral valve area measurements.

Background:

  • The relationship between mitral valve area (MVA) and exercise capacity in mitral stenosis (MS) remains unclear.
  • Previous studies using invasive or 2D-echo methods have yielded controversial results.

Purpose of the Study:

  • To investigate the correlation between Doppler-derived mitral valve indices and exercise capacity in patients with mitral stenosis.
  • To determine if Doppler echocardiography can predict functional limitations in MS.

Main Methods:

  • Nineteen MS patients underwent Doppler echocardiography (measuring MVA, PHT, MPG) and symptom-limited exercise testing (assessing Watts, METs, FAI, MAI).
  • Correlation coefficients (r) were calculated between Doppler parameters and exercise capacity metrics.

Main Results:

  • No significant correlation was found between Doppler-estimated MS severity and patient effort capacity.
  • A weak negative correlation (r = -0.32) was observed between effort intensity and mean pressure gradient (MPG).
  • A weak positive correlation (r = +0.41) was noted between functional aerobic impairment (FAI) and pressure half time (PHT).

Conclusions:

  • Doppler-derived indices of mitral stenosis severity do not reliably predict a patient's exercise capacity.
  • Disease severity in mitral stenosis should be assessed using established methods beyond exercise testing.
