Summary

Posterolateral diaphragmatic hernia surgery in children had a 50% mortality rate. Follow-up revealed good physical condition but reduced lung function due to lung hypoplasia and diaphragm impairment.

Background:

Posterolateral diaphragmatic hernia (PDH) is a serious congenital condition.
Historically, surgical outcomes for PDH have been challenging.

Purpose of the Study:

To evaluate the long-term outcomes of surgically treated posterolateral diaphragmatic hernia.
To assess physical condition and pulmonary function in patients post-PDH repair.

Main Methods:

Retrospective analysis of 125 pediatric cases operated between 1965-1974.
Follow-up included physical examinations, spirometry, and diaphragm mobility studies.

Main Results:

Overall operative mortality was 50%.

Follow-up in 19 cases showed excellent physical condition.

Pulmonary function tests revealed moderate (20-30%) lung function reduction.

Conclusions:

Despite good physical outcomes, long-term pulmonary deficits persist post-PDH repair.
Lung hypoplasia and impaired diaphragm movement contribute to reduced lung function.
Neonatal condition and age-related lung function decline are critical factors in later life.
