Neuroimaging rarely reveals serious pathology in patients with typical headaches or migraines, even with a normal neurological exam. However, "first or worst" headaches require prompt evaluation for subarachnoid hemorrhage, as CT scan sensitivity decreases rapidly post-ictus.
