Myelofibrosis can cause painful bone inflammation called periostitis. Radiation therapy effectively relieved pain in patients with severe symptoms, suggesting a link to increased bone blood flow.

    Area of Science:

    • Hematology
    • Oncology
    • Radiology

    Background:

    • Myelofibrosis (MF) is a chronic bone marrow disorder characterized by fibrosis and extramedullary hematopoiesis.
    • Patients with MF can develop various complications, including bone pain and skeletal abnormalities.
    • Periostitis, inflammation of the periosteum, is a less common but potentially debilitating manifestation.

    Observation:

    • Two patients with myelofibrosis presented with fever, leg pain, and periostitis.
    • Radiographic and bone scan findings confirmed periosteal new bone formation and increased uptake in affected bones.
    • One patient experienced periostitis in both lower and upper extremities, while the other had tibial and fibular involvement.

    Findings:

    • 99mTechnetium pyrophosphate bone scans revealed increased tracer uptake in the inflamed periosteal regions of both patients.

    Related Experiment Videos

  • The observed periostitis may be associated with increased bone blood flow, a known phenomenon in myelofibrosis.
  • Radiation therapy to the affected lower extremities provided significant pain relief for the first patient.
  • Implications:

    • Periostitis should be considered in the differential diagnosis of bone pain in patients with myelofibrosis.
    • Radiation therapy can be an effective palliative treatment for symptomatic periostitis in myelofibrosis.
    • Further research into the pathophysiology of bone blood flow abnormalities in myelofibrosis is warranted.
