Background:

Primary myelofibrosis is a bone marrow disorder.
Bone pain is a reported symptom in myelofibrosis patients.

Observation:

An 80-year-old male presented with severe bone pain in lower limbs and feet.
Initial diagnosis was primary myelofibrosis with osteoporosis.
Pain recurred and worsened, coinciding with leukemic transformation.

Findings:

Autopsy revealed microfractures and osteoporosis.
Leukemic cell infiltration in the periosteum of the tibia was identified as a likely cause of bone pain.
This finding suggests a direct link between leukemic infiltration and skeletal pain in myelofibrosis.
