Summary

Norepinephrine levels in benign prostatic hyperplasia (BPH) tissue differ significantly between histological subtypes, with fibromuscular types showing higher content. Age and symptom severity did not correlate with norepinephrine levels in BPH.

Background:

  • Benign prostatic hyperplasia (BPH) is a common condition in aging men.
  • The role of neurotransmitters, such as norepinephrine, in BPH pathophysiology is not fully understood.
  • Histological variations within BPH may influence tissue composition and biochemical markers.

Purpose of the Study:

  • To determine tissue norepinephrine content in different histological subtypes of BPH.
  • To investigate correlations between norepinephrine levels and patient age, prostate weight, and clinical symptom scores.

Main Methods:

  • Norepinephrine content was measured in prostate tissue specimens from 28 patients undergoing transurethral resection.
  • Specimens were histologically classified into fibromyoadenomatous hyperplasia (18 cases) and fibromuscular type (10 cases).

  • Statistical analysis was performed to compare norepinephrine levels and assess correlations.
  • Main Results:

    • Significantly lower norepinephrine content was found in fibromyoadenomatous BPH (133.1 ± 23.1 ng/g) compared to fibromuscular BPH (340.3 ± 60.5 ng/g) (p < 0.001).
    • Norepinephrine levels showed no correlation with age or clinical symptom score in either group.
    • A reverse correlation between norepinephrine content and prostatic weight was observed exclusively in the fibromyoadenomatous subtype (p < 0.05).

    Conclusions:

    • Tissue norepinephrine levels in BPH are dependent on histological subtype, particularly the proportion of smooth muscle.
    • The fibromuscular subtype of BPH exhibits higher norepinephrine content.
    • Neither patient age nor the severity of prostatism correlates with tissue norepinephrine levels.
