Magnetic resonance imaging (MRI) is crucial for diagnosing radiation optic neuropathy (RON). Gadolinium-enhanced MRI clearly shows lesions in the visual pathways, helping rule out tumor recurrence.

Area of Science:

  • Neurology
  • Radiology
  • Oncology

Background:

  • Radiation optic neuropathy (RON) is a rare complication of radiotherapy.
  • It is caused by obliterative endarteritis and requires ruling out tumor recurrence.

Observation:

  • Five patients with suspected RON underwent MRI in 1992.
  • MRI visualized radiation-induced lesions in the intracranial visual pathways.

Findings:

  • Lesions appeared as sectorial swelling without Gadolinium, markedly enhancing with it.
  • Intracranial optic nerve (5/5), optic chiasm (3/5), and optic tract (2/5) were affected.

Implications:

  • MRI is the preferred diagnostic tool for suspected RON.
  • Gadolinium-enhanced MRI showing segmental swelling suggests radionecrosis.
