Long-acting injectable Sandostatin-LAR effectively reduced growth hormone (GH) and normalized insulin-like growth factor-1 (IGF-1) in acromegaly patients. This treatment improved symptoms without increasing adverse events compared to subcutaneous administration.

Area of Science:

  • Endocrinology
  • Oncology
  • Pharmacology

Background:

  • Acromegaly, characterized by excess growth hormone (GH), poses treatment challenges with surgery and radiotherapy having limitations.
  • Evaluating treatment efficacy for acromegaly is difficult due to a lack of consensus on defining a cure.
  • Persistent GH hypersecretion and delayed effects of radiotherapy necessitate interest in pharmacological interventions.

Purpose of the Study:

  • To evaluate the efficacy and safety of Sandostatin-LAR in managing acromegaly.
  • To assess the impact of Sandostatin-LAR on GH and IGF-1 levels, as well as clinical symptoms.

Main Methods:

  • Eight patients with biochemically diagnosed acromegaly (GH non-suppression post-glucose tolerance test) were treated.
  • Sandostatin-LAR was administered intramuscularly at doses of 20-30 mg every 4 weeks.

Related Experiment Videos

  • Serum octreotide, GH, and IGF-1 levels were monitored, along with clinical signs and symptoms.
  • Main Results:

    • Intramuscular Sandostatin-LAR achieved consistent therapeutic octreotide levels.
    • GH secretion was suppressed to ≤ 5 µg/L in all patients.
    • IGF-1 levels were lowered in all patients, with normalization in seven out of eight.
    • Clinical symptoms and signs of acromegaly improved or resolved.
    • No increase in adverse events was observed compared to subcutaneous treatment.

    Conclusions:

    • Intramuscular Sandostatin-LAR is an effective and well-tolerated treatment for acromegaly.
    • This formulation offers a viable pharmacological option for managing GH excess and its associated symptoms.
    • Sandostatin-LAR provides a consistent and therapeutic approach to acromegaly management.
