Background:

  • Motor Neuron Disease (MND) is a progressive neurodegenerative disorder.
  • Central motor conduction time (CMCT) is a key electrophysiological measure.

Purpose of the Study:

  • To evaluate central motor conduction time (CMCT) in patients with motor neuron disease (MND).
  • To characterize the pattern of CMCT abnormalities in upper and lower limbs.
  • To correlate CMCT findings with the underlying pathophysiology of MND.

Main Methods:

  • Central motor conduction time (CMCT) was measured to the abductor digiti minimi (ADM) and tibialis anterior (TA) muscles.
  • Electrophysiological assessments were performed on 21 patients diagnosed with MND.
  • Data on excitability and conduction time were analyzed for both upper and lower limbs.

Main Results:

  • Motor pathways were inexcitable in a significant proportion of patients (13/21 upper limb, 10/21 lower limb).
  • Prolonged CMCT was observed in 7/21 (ADM) and 14/21 (TA) sides.
  • Abnormalities were asymmetric and randomly focal, not following a constant pattern.

Conclusions:

  • Asymmetric and focal CMCT abnormalities in MND patients support the concept of asymmetric and focal neuronopathy.
  • Electrophysiological findings provide insights into the distribution of neuronal damage in MND.
  • CMCT measurements are valuable for characterizing the neurophysiological impact of MND.
