Summary

Renal cholesterol embolization, a known condition, rarely affects kidney transplant recipients. This report details two cases of primary renal allograft nonfunction caused by cholesterol emboli originating from the donor aorta.

Area of Science:

  • Nephrology
  • Transplant Surgery
  • Vascular Pathology

Background:

  • Renal cholesterol embolization is a recognized clinical entity.
  • It is infrequently documented in renal transplant recipients.

Observation:

  • Two cases of primary renal allograft nonfunction are presented.
  • Cholesterol emboli were identified as the cause of nonfunction.
  • The presumed source of emboli was the donor aorta.

Findings:

  • Cholesterol embolization can lead to primary nonfunction of renal allografts.
  • Donor aorta atherosclerosis is a potential source of these emboli.

Implications:

  • Cholesterol embolization should be considered in the differential diagnosis of early renal allograft dysfunction.
  • Careful donor aorta evaluation and procurement techniques may mitigate this complication.
