Summary
Benign monocytoid B-cells (MBCs) proliferation, previously not seen in peripheral T-cell lymphomas (PTCL), was identified in PTCL cases. MBCs may represent a diminishing residual component of prior hyperimmune reactions in PTCL of angioimmunoblastic lymphadenopathy type.

Background:
- Benign monocytoid B-cells (MBCs) proliferation is typically observed in various lymphoproliferative disorders.
- Its presence in peripheral T-cell lymphomas (PTCL) has not been previously reported.
Observation:
- A review of 53 nodal PTCL cases identified MBCs in 8 cases.
- MBCs were found in chronic lymphocytic leukemia of T-cell type, anaplastic large cell lymphoma, and PTCL of angioimmunoblastic lymphadenopathy (AILD) type.
- Progression from AILD-hyperimmune reaction to PTCL of AILD type was observed in one case.
Findings:
- In PTCL of AILD type, MBCs appeared as solitary or multifocal clusters, often diminishing with disease progression.
- MBCs in PTCL of AILD type likely represent a residual component of a preceding hyperimmune reaction.
- Cytologically, MBCs in PTCL exhibited common and large cell types, consistent with reactive lymphadenopathies.
Implications:
- The findings suggest that PTCL of AILD type may develop from preceding hyperimmune reactions more frequently than previously thought.
- This challenges the established understanding of MBCs' role in lymphoproliferative disorders.
- Further research into the relationship between hyperimmune reactions and PTCL development is warranted.
