Summary

Transtracheal jet ventilation (TJV) at frequencies above 80 breaths/min prevents aspiration. Discontinuation requires clearing the airway above the jetting port to prevent pulmonary aspiration due to pressure changes.

Background:

  • Transtracheal jet ventilation (TJV) is used for difficult airways.
  • Controversies exist regarding TJV and pulmonary aspiration risk.
  • Previous studies suggest aspiration prevention at >60 breaths/min or even lower with specific techniques.

Purpose of the Study:

  • To reevaluate the controversy surrounding pulmonary aspiration during TJV.
  • To observe both the trachea and lungs simultaneously during TJV.

Main Methods:

  • Eight anesthetized and paralyzed dogs were ventilated using TJV at varying frequencies (10-600/min).
  • Airway pressures were measured.
  • Barium was introduced into the mouth, and chest X-rays were taken to detect aspiration.

Main Results:

  • No tracheal or pulmonary aspiration was observed on X-rays when TJV frequencies were above 80 breaths/min.
  • At 60 breaths/min, barium was present in the trachea but above the jetting port.
  • Lowering frequency to 10 breaths/min did not alter barium levels.

Conclusions:

  • No aspiration was visualized on chest X-rays.
  • Forceful gas outflow through the larynx likely prevents aspiration.
  • Limited tracheal aspiration may be due to negative upper airway pressure.
  • Clearing the airway above the jetting port before discontinuing TJV is recommended to prevent aspiration.
