    Summary

    Large vessel impedance (ZL) effectively measures arterial stenosis severity in humans, independent of small vessel resistance. This finding validates animal model results in clinical atherosclerosis research.

    Background:

    • Animal studies indicate large vessel impedance (ZL) correlates with stenotic resistance, independent of distal arteriolar resistance.
    • Atherosclerotic disease commonly affects large vessels, leading to stenosis and altered hemodynamics.

    Purpose of the Study:

    • To assess the applicability of large vessel impedance measurements in human subjects with atherosclerotic lesions.
    • To evaluate ZL as an index for quantifying the severity of femoral arterial stenosis in patients.

    Main Methods:

    • Determined ZL in the femoral arteries of 17 patients with varying degrees of atherosclerotic disease (none, mild, severe).
    • Measured ZL under steady-state conditions and during peak flow after papaverine hydrochloride administration.
    • Compared ZL before and after femoral-to-popliteal bypass procedures.

    Main Results:

    • Significantly higher ZL in patients with severe atherosclerotic disease compared to healthy controls.
    • No significant difference in ZL between steady-state and papaverine-induced peak flow conditions.
    • Significant decrease in ZL post-femoral-to-popliteal bypass, indicating improved large vessel characteristics.

    Conclusions:

    • Large vessel impedance is a reliable index for assessing the severity of large vessel atherosclerotic stenosis in humans.
    • ZL measurements are independent of small vessel resistance in human subjects.
    • Findings support the translation of animal model results to clinical applications in vascular disease.
